MentalNote (Mental Note AI) is an AI scribe for clinicians in solo and small practices. In the browser web app, a clinician records a visit with consent, and when the session ends the tool produces a structured draft note with a speaker-separated transcript. Drafts are editable and can be copied or exported as PDF. An optional Microsoft Word add-in, installed from Microsoft AppSource, drafts notes from a typed or pasted session summary instead.
Supported formats include SOAP, DAP, BIRP, GIRP, PIRP and Progress notes, plus Intake and Treatment Plan workflows and a Medical SOAP format for primary-care follow-ups. The product does not write back directly to EHRs; finished notes are copied or exported to the practice's system of record. A Business Associate Agreement is included on every plan and accepted in the app before the first session.
